Personalized Care

St. Luke’s Hospice At Home Care is a solution for families who need that extra special level of personal service. Beginning with a complimentary assessment by one of our experienced Life Care Managers, our Professional Staff provides expert and compassionate care for those adults who are coping with a variety of afflictions including:

  • Alzheimer’s, Parkinson’s, Lewy body and other types of dementia.
  • Recovery after serious injury or surgery
  • Limited mobility
  • Chronic illnesses

 

Professional Staff

At St. Luke’s Hospice, care is carefully planned and guided by our highly-qualified Life Care Managers. Personal attendants are available – at any time for any duration – whether it is for just an hour or two as needed or if full-time live-in or 24-hour around the clock care is required.
Caregivers are able to supply basic care, help with meal preparation, companionship, light housekeeping, medication reminders, and personal hygiene assistance. It is important to know that all caregivers at St. Luke’s Hospice are employees who have undergone thorough and comprehensive background checks.

Our personal attendants are expertly-trained and focused on providing professional personalized care that adheres to the highest industry standards. Because personal interaction and trust are essential to effective At Home Care, our staff is screened and our personnel are matched with a client’s needs and preferences. The personality of the caregiver is often just as important as their skills and abilities as they must have some type of chemistry with the patient. It should be noted that Respite Care at a St. Luke’s Memory Care Community is available to all At Home Care patients.

Service with a Purpose

At St. Luke’s Hospice we believe that the way a person leaves this planet is just as important as how they spent their time on it. Our skilled and professional staff sees Hospice Care as the exclamation mark at the end of a life well-lived; inspiring a work ethic that provides non-stop care for the patient to help comfort their mind, body and soul.

By delivering personalized medical, social and spiritual assistance, our Hospice Care team gives patients a chance to experience life to the fullest and provides their families with an opportunity to create cherished and lasting memories.

 

Attending Physician

Attending Physicians at St. Luke’s Hospice oversee medical care and provide prescriptions to help manage a patient’s pain and symptoms. Sometimes the Medical Director will serve as the Attending Physician.

Hospice Medical Directors

St. Luke’s Hospice Medical Directors serve as liaisons between the attending physician, the patient’s family and the patient. The family often provides vital information and valuable insight pertaining to the patient’s needs.

Social Worker

Social Workers at St. Luke’s Hospice not only advise patients and families when needed, but they develop an integrated Hospice Care plan based on a patient’s emotional, financial, physical and social needs. Social Workers also help with bereavement support after a loved one has passed.

Spiritual Counselor

Spiritual Counselors at St. Luke’s Hospice employ the values and beliefs of patients and their families to help cope with the spiritual facets of illness and loss. Spiritual Counselors can also help with specific religious requirements and lead memorial services if needed.

Volunteers

The Volunteers at St. Luke’s Hospice offer support for all non-medical needs like providing companionship and insuring the patient is being socially active, assisting with family needs and running errands.

Hospice Aides (CNA/CHHA)

Certified Nursing Assistants and Certified Home Health Aides at St. Luke’s Hospice provide personal care to each patient, report any changes in need and offer comforting measures for patients and their families as directed.

RN Case Manager

At St. Luke’s Hospice, the Registered Nurse Case Managers are responsible for coordinating all aspects of the integrated Hospice Care plan for the patient while supervising symptom control, pain management and the administering of medications as needed.
